Generic Information: Shiyao (China)
Shiyao (石药/瑞戈非尼) is the CSPC Ouyi brand of regorafenib, manufactured by CSPC Ouyi Pharmaceutical Co., Ltd., part of one of China's largest pharmaceutical groups. This pack contains 28 film-coated tablets of 40 mg. As a domestic Chinese regorafenib, it provides a licensed, more accessible option for patients whose advanced cancer requires later-line multi-kinase inhibition.
What Is Regorafenib (Shiyao) Used For?
Shiyao is used to treat three advanced cancers. It is indicated for metastatic colorectal cancer that has progressed after standard chemotherapy plus anti-VEGF and, where applicable, anti-EGFR therapy; for gastrointestinal stromal tumour resistant to imatinib and sunitinib; and for hepatocellular carcinoma that has progressed on sorafenib. It is a disease-control therapy given after earlier treatments have failed.
How Does Regorafenib Work?
Regorafenib inhibits multiple kinases central to tumour growth and angiogenesis, including the VEGF receptors 1, 2 and 3, TIE2, PDGFR-α and β, FGFR-1 and 2, KIT, RET, RAF-1 and BRAF. This dual effect — slowing cancer-cell division while blocking new blood-vessel formation — is what lets a single drug work against colorectal cancer, gastrointestinal stromal tumour and hepatocellular carcinoma.
Indications
The approved indications are advanced colorectal cancer after prior standard therapy, imatinib- and sunitinib-resistant gastrointestinal stromal tumour, and sorafenib-refractory hepatocellular carcinoma. Treatment must be supervised by an oncologist, with liver function and blood pressure evaluated before and during therapy.
Dosage
The standard dose is 160 mg, taken as four 40 mg tablets once daily with a low-fat meal, in 28-day cycles of 21 days on and 7 days off. A 28-tablet pack supplies seven days of the active-treatment phase. The oncologist may reduce the dose to 120 mg or 80 mg daily if hand-foot skin reaction, diarrhoea or liver enzyme elevations occur.
Side Effects
Common side effects include hand-foot skin reaction, diarrhoea, fatigue, hypertension, decreased appetite and mucositis. More serious adverse events can include liver injury, bleeding, gastrointestinal perforation and cardiac ischaemia. Regular checks of liver function, blood pressure and blood counts are recommended, and patients should promptly report skin peeling, persistent diarrhoea, dark stools or chest pain.
Storage
Store Shiyao tablets at room temperature, protected from moisture, in the original packaging and out of the reach of children. Do not use beyond the expiry date printed on the pack.
