For AML in combination with azacitidine, ramp up 100 mg → 200 mg → 400 mg over cycle 1 days 1–3, then 400 mg daily. Tablets are taken orally with food and water, swallowed whole, at the same time each day. Treatment must be initiated and supervised by an experienced hematologist/oncologist.

Side Effects

The most clinically important risk is tumor lysis syndrome (TLS) during initiation in high-tumour-burden patients. Common adverse reactions include neutropenia (with infection risk), diarrhoea, nausea, anaemia, thrombocytopenia, fatigue and cough. Strong CYP3A4 inhibitors/inducers require dose adjustment or avoidance. Venetoclax can cause fetal harm; effective contraception is required during and after treatment.
