Selpercatinib is a highly selective RET kinase inhibitor: it binds and blocks RET, shutting down downstream signaling that fuels tumor growth. Because it crosses the blood-brain barrier, it is effective against brain metastases. In the LIBRETTO-001 trial, 10 of 11 previously treated NSCLC patients with measurable brain metastases showed intracranial responses.

Dosage
The recommended dose in adults and adolescents 12 years and older is based on body weight: 120 mg orally twice daily for patients under 50 kg, and 160 mg orally twice daily for patients 50 kg or greater, until disease progression or unacceptable toxicity. Dose reductions are required for severe hepatic impairment.
Side Effects
The most common side effects of selpercatinib include dry mouth, diarrhea, hypertension, fatigue, edema, constipation, rash, and elevated liver enzymes (ALT/AST). Serious side effects include hepatotoxicity, interstitial lung disease (ILD)/pneumonitis, hypertension, QT interval prolongation, hemorrhagic events, and hypersensitivity reactions. Liver function and blood pressure should be monitored regularly during treatment.
Storage
Store selpercatinib at room temperature (20°C to 25°C / 68°F to 77°F). Keep in the original container, tightly closed, and out of reach of children. Do not use after the expiration date printed on the package.
