Zykadia is the originator brand of ceritinib, developed and marketed by Novartis of Switzerland. This Swiss-origin presentation comes as a 10-capsule starter pack, with each capsule containing 150 mg of ceritinib. Zykadia was the first second-generation ALK inhibitor approved for patients with ALK-positive non-small cell lung cancer that had progressed despite treatment with crizotinib.

What Is Zykadia Used For?

Zykadia treats metastatic non-small cell lung cancer driven by an ALK gene rearrangement. It is prescribed for patients who have already received crizotinib and whose cancer has advanced, and it has also gained approval as a first-line option for newly diagnosed ALK-positive disease. How Does Ceritinib (Zykadia) Work?

Ceritinib is a selective inhibitor of anaplastic lymphoma kinase. ALK is an enzyme that, when abnormally activated by a gene rearrangement, sends continuous growth signals that drive lung cancer. Ceritinib blocks ALK and, in doing so, also overcomes many of the secondary ALK mutations that render tumours resistant to first-generation crizotinib, giving patients a meaningful second chance after progression.

Indications

Zykadia is indicated for ALK-positive metastatic NSCLC, as confirmed by a validated diagnostic test. It is used both after crizotinib failure and as initial therapy in patients with this molecular subtype. Treatment should be initiated and supervised by an oncologist experienced in targeted lung-cancer therapy.

Dosage

The standard regimen is 450 mg (three 150 mg capsules) once daily with food, which achieves the same exposure as the earlier 750 mg fasting dose with better gastrointestinal tolerability. The 10-capsule starter pack covers just over three days at this dose, making it suitable for treatment initiation, dose titration, or a bridging supply before a larger pack arrives.

Side Effects

Common adverse reactions include diarrhoea, nausea, vomiting, abdominal pain, fatigue and decreased appetite. Clinically important effects include elevations of liver enzymes, hyperglycaemia, QT-interval prolongation, bradycardia and, rarely, interstitial lung disease or pneumonitis. Patients undergo regular monitoring of liver function, blood glucose and the ECG, and should report any persistent digestive symptoms or new respiratory difficulty.

Storage

Keep Zykadia capsules at room temperature, away from moisture and direct light, in the original packaging and out of reach of children. Do not use beyond the expiry date printed on the pack.
