What Is Ribociclib (HER-2N) Used For?
HER-2N is used to treat HR-positive, HER2-negative breast cancer. In advanced or metastatic disease it is combined with an aromatase inhibitor or fulvestrant, and it is also indicated for adjuvant treatment of HR-positive, HER2-negative stage II/III early breast cancer at high risk of recurrence. Despite the brand name, the medicine targets HR-positive, HER2-negative disease, and it is always used together with endocrine therapy rather than alone.
How Does Ribociclib Work?
Ribociclib selectively inhibits cyclin-dependent kinases 4 and 6, which govern the G1-to-S checkpoint of the cell cycle. HR-positive breast-cancer cells rely on CDK4/6 signalling to continue dividing, so blocking these kinases halts them at the checkpoint and slows tumour growth. Paired with hormone therapy that suppresses oestrogen-driven growth, the combination slows the disease from two complementary directions.
Indications
The approved indications are HR-positive, HER2-negative advanced or metastatic breast cancer in combination with an aromatase inhibitor or fulvestrant, and HR-positive, HER2-negative stage II/III early breast cancer at high risk of recurrence as adjuvant therapy. Treatment should be initiated and monitored by an oncologist, and hormone-receptor and HER2 status must be confirmed before starting.
Dosage
The standard dose for advanced or metastatic disease is 600 mg (three 200 mg tablets) once daily, with or without food, in 28-day cycles of 21 days on and 7 days off. A 21-tablet pack covers one week of active dosing. For early breast cancer the dose is 400 mg daily. The dose may be reduced to 400 mg or 200 mg daily to manage neutropenia, liver-enzyme elevations or QT prolongation.
Side Effects
Common side effects include neutropenia, nausea, fatigue, diarrhoea, vomiting, hair loss, headache and constipation. More serious adverse events can include QT-interval prolongation, liver toxicity, interstitial lung disease and severe skin reactions. Blood counts, liver function, electrolytes and the ECG are monitored regularly, and patients should report fever, persistent diarrhoea, yellowing of the skin or new breathing difficulty promptly.
Storage
Store HER-2N tablets at room temperature in the original container, protected from moisture, and out of the reach of children. Do not use past the expiry date on the pack.
