Generic Information: Calquence (Acalabrutinib) 100mg — United States

Calquence is the originator brand of acalabrutinib developed by AstraZeneca, a highly selective, second-generation covalent inhibitor of Bruton's tyrosine kinase (BTK). It was first approved by the FDA in 2017 for mantle cell lymphoma and has since become a standard targeted therapy for several B-cell blood cancers. This US listing is the 100 mg tablet in a 60-tablet pack, the standard presentation for the twice-daily dosing regimen.

Acalabrutinib was designed for greater BTK selectivity than the first-generation ibrutinib, with the aim of reducing off-target effects on kinases such as EGFR and ITK. For patients and institutions seeking the originator US product, Calquence represents the reference standard against which all generics and biosimilars are measured.

What Is Calquence Used For?

Calquence is indicated for three B-cell malignancies. It treats adult patients with mantle cell lymphoma (MCL) who have received at least one prior therapy, and — in combination with bendamustine and rituximab — adults with previously untreated MCL who are ineligible for autologous stem cell transplant. It is also indicated for chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL).

How Does Calquence Work?

Acalabrutinib binds covalently to a cysteine residue in the active site of BTK, irreversibly blocking the kinase. BTK sits downstream of the B-cell receptor and is essential for the survival, proliferation and migration of malignant B cells. By shutting down BTK signalling, Calquence suppresses the growth of the cancerous B-cell population that drives MCL, CLL and SLL.

Indications

  • Mantle cell lymphoma (MCL), relapsed or refractory, after at least one prior therapy.
  • Previously untreated MCL, in combination with bendamustine and rituximab, for patients ineligible for autologous stem cell transplant.
  • Chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Dosage and Administration

The recommended dose is 100 mg taken orally approximately every 12 hours, swallowed whole with water, with or without food. Tablets must not be chewed, crushed, dissolved or cut. Treatment continues until disease progression or unacceptable toxicity. Dose adjustments are required with strong or moderate CYP3A inhibitors, and Calquence should be avoided in severe hepatic impairment.

Side Effects

The most common adverse reactions include upper respiratory tract infection, diarrhoea, headache and musculoskeletal pain. Clinically important risks include serious infections, bleeding events, low blood counts (cytopenias), second primary malignancies (including skin cancer), cardiac arrhythmias such as atrial fibrillation, and liver injury. Regular blood counts and liver function monitoring are recommended throughout therapy.

Storage

Store Calquence at room temperature (below 30°C) in the original container, protected from moisture and out of the reach of children. Do not use after the expiry date printed on the pack.
