Generic Information: Promacta (United States)
Generic name: eltrombopag olamine. Brand name: Promacta. Manufacturer: Novartis. Origin: United States (originator). Strength: 25 mg and 50 mg film-coated tablets, 30 tablets per bottle. Class: thrombopoietin receptor agonist (TPO-RA), a non-peptide small molecule that binds the transmembrane domain of the c-Mpl receptor.
Promacta is the United States brand of the originator eltrombopag, first approved by the FDA in 2008 as the first oral TPO receptor agonist for immune thrombocytopenia. The identical molecule is sold as Revolade in the European Union and China. Both are manufactured by Novartis, which has held global rights to eltrombopag since 2015.
What Is Promacta Used For?
Promacta raises platelet counts in two core indications. First, chronic immune thrombocytopenia (ITP) in adults and children aged 1 year and older who have not responded adequately to corticosteroids, immunoglobulins or splenectomy. Second, severe aplastic anemia (SAA) in patients with an insufficient response to immunosuppressive therapy. In the United States it is also approved for thrombocytopenia associated with chronic hepatitis C to allow interferon-based treatment.
How Does Promacta Work?
Eltrombopag binds directly to the transmembrane region of the thrombopoietin receptor (c-Mpl) on megakaryocytes, triggering JAK2/STAT signalling that drives megakaryocyte proliferation, differentiation and platelet release. Because it binds a site distinct from where endogenous thrombopoietin binds, Promacta works additively with the body's own TPO rather than competing with it. It is the reference oral therapy that spared patients the daily injections of earlier recombinant TPO products.
Indications
- Chronic immune thrombocytopenia (ITP) in adults and children ≥1 year with inadequate response to corticosteroids, IVIG or splenectomy
- Severe aplastic anemia (SAA) with inadequate response to immunosuppressive therapy
- Thrombocytopenia associated with chronic hepatitis C, to enable interferon-based therapy
Dosage
For ITP, adults and children 6 years and older usually start at 50 mg once daily, while children 1–5 years start at 25 mg once daily; the dose is titrated to maintain platelets at or above 50 × 10⁹/L, not exceeding 75 mg daily. For SAA, the typical starting dose is 50 mg once daily for adults. East Asian patients and those with hepatic impairment start at a lower dose.
Promacta must be taken on an empty stomach — one hour before or two hours after a meal — and separated by at least two hours from dairy products and supplements containing calcium, iron, magnesium, aluminium, selenium or zinc, which sharply reduce absorption.
Side Effects
The most common adverse reactions are headache, nausea, diarrhoea, fatigue, muscle aches and upper respiratory symptoms. More serious risks require monitoring: hepatotoxicity (liver enzymes are checked before and periodically during treatment), thromboembolic events, and cataract formation. Regular platelet counts and liver function tests are essential while on therapy.
Storage
Store Promacta tablets at room temperature (20–25°C), in a dry place away from light and moisture. Keep the bottle tightly closed and out of reach of children.
