Generic Information: Afinitor (Everolimus) — International Originator

Everolimus is the active ingredient of Afinitor, the originator mTOR (mammalian target of rapamycin) inhibitor developed and marketed by Novartis. This page covers the international originator presentation supplied in three tablet strengths — 2.5 mg, 5 mg, and 10 mg — each in a 30-tablet pack. Afinitor has been approved by the U.S. FDA since 2009 and by regulatory authorities worldwide, making it the most widely referenced everolimus brand across oncology practice. The three-strength range supports flexible dose titration, which is particularly important because everolimus requires therapeutic drug monitoring to keep trough blood concentrations within the 5–15 ng/mL window.

What Is Afinitor (Everolimus) Used For?

Afinitor is a multi-indication kinase inhibitor approved for several cancers and tuberous sclerosis complex (TSC)-related conditions. Its principal uses are advanced renal cell carcinoma (RCC) that has progressed after treatment with sunitinib or sorafenib, progressive pancreatic neuroendocrine tumors (PNET), and progressive, well-differentiated, non-functional neuroendocrine tumors (NET) of gastrointestinal or lung origin. It is also indicated for hormone receptor-positive, HER2-negative advanced breast cancer in combination with exemestane, and for TSC-associated renal angiomyolipoma and subependymal giant cell astrocytoma (SEGA).

How Does Afinitor (Everolimus) Work?

Everolimus binds the intracellular protein FKBP-12 to form a complex that selectively inhibits mTOR complex 1 (mTORC1). This blockade reduces the activity of downstream effectors S6K1 and 4E-BP1, which in turn slows protein synthesis, cell proliferation, angiogenesis, and glucose metabolism in tumor cells. Because the mTOR pathway is frequently overactive in renal, neuroendocrine, and breast tumors, interrupting it produces durable tumor control. Everolimus also has immunosuppressive activity, which is why the related brand Zortress/Certican is used to prevent organ transplant rejection.

Indications

Afinitor is indicated for: (1) advanced renal cell carcinoma after failure of sunitinib or sorafenib; (2) progressive pancreatic neuroendocrine tumors; (3) progressive, well-differentiated, non-functional GI or lung neuroendocrine tumors; (4) hormone receptor-positive, HER2-negative advanced breast cancer combined with exemestane; (5) TSC-associated renal angiomyolipoma not requiring immediate surgery; and (6) TSC-associated subependymal giant cell astrocytoma that cannot be curatively resected.

Dosage

The recommended dose is 10 mg once daily for RCC, NET, breast cancer, and TSC-associated angiomyolipoma. For TSC-associated SEGA, the starting dose is 4.5 mg/m² once daily, titrated to a target trough of 5–15 ng/mL. The tablet may be taken with or without food, but it must be swallowed whole with water and taken at the same time each day. Dose reduction to 5 mg or 2.5 mg once daily may be required for hepatic impairment or interacting medications, which is why all three strengths are stocked here.

Side Effects

The most common adverse reactions include stomatitis, rash, fatigue, diarrhea, infections, nausea, decreased appetite, peripheral edema, hyperglycemia, and dyslipidemia. Serious but less common events include non-infectious pneumonitis, renal failure, delayed wound healing, and severe hypersensitivity reactions. Regular monitoring of kidney function, blood counts, fasting lipids, and blood glucose is recommended throughout treatment.

Storage

Store Afinitor tablets in the original blister packaging at room temperature (20–25°C), protected from moisture and light. Keep out of reach of children. Do not use tablets after the expiry date printed on the carton.

FAQ

What strengths of Afinitor are available? Afinitor is supplied in 2.5 mg, 5 mg, and 10 mg tablets, each in a 30-tablet pack, supporting the dose adjustments required during treatment.

Can Afinitor be taken with food? Yes, Afinitor may be taken with or without food, but it should be taken at the same time each day and swallowed whole with water.

What is the difference between Afinitor and Zortress? Both contain everolimus, but Afinitor is used in oncology while Zortress (Certican in the EU) is used to prevent organ transplant rejection at much lower doses.

Why does everolimus need blood concentration monitoring? Because its metabolism is affected by liver function and interacting drugs, monitoring trough levels (target 5–15 ng/mL) helps keep dosing safe and effective.
