Subeli (苏贝利) is a Chinese-market brand of regorafenib manufactured by Qilu Pharmaceutical Co., Ltd., one of China's largest oncology and specialty pharmaceutical companies. What Is Regorafenib (Subeli) Used For?

Subeli is used to treat metastatic colorectal cancer that has progressed after standard chemotherapy plus anti-VEGF and, where appropriate, anti-EGFR therapy. It is also indicated for gastrointestinal stromal tumour resistant to imatinib and sunitinib, and for hepatocellular carcinoma that has progressed on sorafenib. Like all regorafenib products, Subeli is a later-line disease-control therapy rather than a first-line treatment.

How Does Regorafenib Work?

Regorafenib blocks a broad set of kinases that cancer cells use to grow and to recruit blood vessels. Its targets include the VEGF receptors 1, 2 and 3, TIE2, PDGFR-α and β, FGFR-1 and 2, KIT, RET, and RAF-1 and BRAF. By inhibiting this network, the drug reduces tumour-cell proliferation and cuts off the blood supply tumours depend on, slowing disease progression in colorectal, stromal-tumour and liver cancers.

Indications

The approved indications are advanced colorectal cancer after prior standard therapy, imatinib- and sunitinib-resistant gastrointestinal stromal tumour, and sorafenib-refractory hepatocellular carcinoma. Treatment should be managed by an oncologist, with liver function and blood pressure checked before and during therapy.

Dosage

The standard dose is 160 mg, taken as four 40 mg tablets once daily with a low-fat meal. Treatment follows 28-day cycles of 21 days on therapy and 7 days off, so a 28-tablet pack covers one week of active dosing. If side effects appear, the oncologist may lower the dose to 120 mg or 80 mg daily and adjust it again as tolerated.

Side Effects

Common adverse reactions include hand-foot skin reaction, diarrhoea, fatigue, hypertension, decreased appetite and mouth sores. Less common but serious effects include liver injury, bleeding, gastrointestinal perforation and cardiac events. Regular monitoring of liver enzymes, blood pressure and blood counts is recommended, and patients should promptly report skin peeling, persistent diarrhoea, dark stools or chest pain.

Storage

Store Subeli tablets at room temperature, protected from moisture, in the original package and out of the reach of children. Do not use beyond the expiry date on the pack.
