Velpatasvir blocks the NS5A protein that organizes viral replication factories inside the host cell. Voxilaprevir inhibits the NS3/4A protease that cleaves the viral polyprotein into functional pieces. Because a prior NS5A-inhibitor failure typically reflects resistance mutations at NS5A rather than a general loss of susceptibility, adding the protease-inhibitor axis restores the ability to clear the virus even in patients carrying common NS5A resistance-associated substitutions. This complementary triple coverage is exactly why the regimen works as a rescue therapy rather than a first-line tablet.

Indications

Vosevi is indicated for the treatment of adult patients with chronic hepatitis C virus (HCV) infection, without cirrhosis or with compensated cirrhosis (Child-Pugh A), who have previously been treated with a DAA-containing regimen: either a regimen that included an NS5A inhibitor (any genotype 1-6), or a sofosbuvir-based regimen without an NS5A inhibitor for patients with genotype 1a or 3. Dosage

The recommended dose is one tablet once daily with food, for a fixed course of 12 weeks. A missed dose should be taken as soon as remembered the same day, but never doubled. No dose adjustment is required for mild or moderate renal impairment — including patients on dialysis — or for mild hepatic impairment. The regimen is not recommended in moderate to severe hepatic impairment (Child-Pugh B or C) because voxilaprevir exposure rises significantly. Prescribers must also screen for contraindicated co-medications before starting, since strong enzyme inducers and several common drugs are incompatible with this combination.

Side Effects

In the registration trials the regimen was remarkably well tolerated: fewer than one in four hundred patients discontinued treatment for adverse events. The most common side effects (reported in at least 10% of patients) were headache, fatigue, diarrhea, and nausea, generally mild to moderate and self-limited. Clinically important warnings include severe symptomatic bradycardia when amiodarone is taken together with sofosbuvir-containing regimens — amiodarone should be avoided — and reactivation of hepatitis B in coinfected patients, which requires monitoring before and during therapy. Efficacy of hormonal contraceptives containing ethinyl estradiol may be compromised, and rosuvastatin or dabigatran must not be co-administered.
