Generic Information: Calquence (Acalabrutinib) 100mg — China-Mainland
Calquence (康可期) is the China-market version of AstraZeneca's originator acalabrutinib, supplied by AstraZeneca Investment (China) Co., Ltd. This listing is the 100 mg tablet in a 56-tablet pack, the presentation registered in China for the BTK inhibitor's approved indications. The drug is a highly selective, second-generation covalent inhibitor of Bruton's tyrosine kinase (BTK).
Acalabrutinib was developed to offer improved BTK selectivity over the first-generation inhibitor, reducing off-target inhibition of kinases such as EGFR and ITK. In China it has been granted conditional approval for adults with CLL/SLL and MCL, and it has since been included in the national medical insurance scheme for the relevant indications.
What Is Calquence Used For?
In China, Calquence is conditionally approved for adult patients with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL), and for adult patients with mantle cell lymphoma (MCL). The drug is used as a single agent for patients who have received at least one prior line of therapy.
How Does Calquence Work?
Acalabrutinib forms a covalent bond with a cysteine residue in the BTK active site, permanently disabling the enzyme. BTK relays signals from the B-cell antigen receptor that malignant B cells depend on for survival and expansion. By blocking this pathway, Calquence reduces the proliferation, migration and adhesion of the cancerous B cells driving CLL, SLL and MCL.
Indications
- Chronic lymphocytic leukemia (CLL) / small lymphocytic lymphoma (SLL) in adults who have received at least one prior therapy.
- Mantle cell lymphoma (MCL) in adults who have received at least one prior therapy.
Dosage and Administration
The recommended dose is 100 mg twice daily, taken approximately every 12 hours. Capsules or tablets are swallowed whole with water, with or without food, and should not be chewed, crushed or opened. If a dose is missed by more than 3 hours, it should be skipped and the next dose taken at the regular time. Dose adjustment is needed with CYP3A inhibitors and in hepatic impairment.
Side Effects
Common adverse reactions include infections (upper respiratory tract, sinusitis, pneumonia), neutropenia, anaemia, thrombocytopenia, headache, dizziness, atrial fibrillation, diarrhoea, nausea, rash and fatigue. Serious risks include bleeding, serious infections, second primary malignancies, cardiac arrhythmias and liver injury. Complete blood counts and liver function should be monitored regularly.
Storage
Store Calquence below 30°C in the original packaging, away from moisture and out of the reach of children. Do not use past the expiry date shown on the pack.
