Generic Information: Zelboraf (Vemurafenib) — Genentech US Originator

Vemurafenib is the active ingredient of Zelboraf, the first-in-class BRAF inhibitor developed and distributed in the United States by Genentech, Inc., a member of the Roche Group. This page covers the 240 mg film-coated tablet supplied in a 56-tablet pack, the same strength used for the standard twice-daily dosing regimen. Zelboraf is a registered trademark of Genentech and is co-promoted with Daiichi Sankyo. It was the first therapy ever approved for BRAF-mutant melanoma, transforming a disease that previously responded poorly to chemotherapy into one with a targeted, biomarker-driven treatment option. The active ingredient is manufactured by F. Hoffmann-La Roche AG, while the finished product carries the Genentech label.

What Is Zelboraf (Vemurafenib) Used For?

Zelboraf is approved for two distinct indications. The first and most established is the treatment of unresectable or metastatic melanoma in patients whose tumors carry the BRAF V600E mutation, confirmed by an FDA-approved companion diagnostic. The second is the treatment of Erdheim-Chester Disease (ECD), an extremely rare blood disorder driven by the same BRAF V600 mutation. This dual approval makes Zelboraf a cornerstone of precision oncology: a single oral tablet that addresses both a common aggressive skin cancer and an ultra-rare histiocytic neoplasm, all guided by a single molecular marker.

How Does Zelboraf (Vemurafenib) Work?

Zelboraf is a selective kinase inhibitor that blocks the mutated BRAF V600E protein. In healthy cells, BRAF relays growth signals along the MAPK pathway in a tightly controlled, on-off fashion. In roughly half of melanomas, a single V600E mutation locks this protein into a permanently active state, causing uncontrolled cell division. Zelboraf binds preferentially to the mutant kinase and switches it off, thereby slowing tumor growth and, in many patients, producing meaningful tumor shrinkage. Because it specifically targets the mutant form, testing for the BRAF V600E mutation is mandatory before treatment; patients with wild-type BRAF must not receive the drug.

Indications

Zelboraf is indicated for: (1) the treatment of patients with unresectable or metastatic melanoma whose tumors harbor the BRAF V600E mutation, as detected by an FDA-approved test; and (2) the treatment of adult patients with Erdheim-Chester Disease with the BRAF V600 mutation. It is not indicated for patients with wild-type BRAF melanoma.

Dosage

The recommended dose of Zelboraf is 960 mg (four 240 mg tablets) taken orally twice daily, approximately 12 hours apart, with or without food. Tablets must be swallowed whole with water and should not be crushed or chewed. Treatment continues until disease progression or unacceptable toxicity. If a dose is missed, it may be taken up to 4 hours before the next scheduled dose. Dose reductions or interruptions are managed according to the severity of adverse reactions, particularly QT prolongation and skin toxicities.

Side Effects

The most common adverse reactions include arthralgia, rash, alopecia, fatigue, photosensitivity, nausea, pruritus, and the appearance of skin papillomas. A hallmark concern is the development of cutaneous squamous cell carcinoma (cSCC), which requires dermatologic evaluation before starting therapy and every two months during treatment. Other serious risks include QT prolongation, hepatotoxicity, severe hypersensitivity reactions such as Stevens-Johnson syndrome, and uveitis. Liver enzymes and electrocardiograms are monitored regularly throughout treatment.

Storage

Store Zelboraf tablets at room temperature between 20°C and 25°C, in the original container with the lid tightly closed. Keep away from moisture and out of the reach of children. Do not use beyond the expiry date printed on the packaging.

Why Choose the GLT?

The United States list price for brand-name Zelboraf is among the highest of any targeted therapy, with a full month of treatment costing well into five figures for cash-paying patients. This places a heavy financial burden on individuals and families facing BRAF-mutant melanoma or Erdheim-Chester Disease. As a Hong Kong-licensed supplier, GLT provides a verified channel for the authentic Genentech product with full batch documentation, transparent sourcing, and a cost structure that is meaningfully more accessible than the Western cash price. Patients and institutions seeking the originator molecule without the American pricing burden can rely on GLT for dependable availability and support.

FAQ

Is Zelboraf the same as vemurafenib? Yes. Zelboraf is the brand name of vemurafenib, developed and distributed in the United States by Genentech, Inc., a member of the Roche Group.

Why is BRAF testing required before using Zelboraf? Zelboraf only works in tumors with the BRAF V600E mutation. In wild-type BRAF tumors the drug can even promote growth, so a molecular test is mandatory before starting treatment.

What is Erdheim-Chester Disease? ECD is an ultra-rare histiocytic neoplasm driven by the BRAF V600 mutation, and Zelboraf is one of the approved targeted treatments for it.

How is Zelboraf taken? It is taken as 960 mg (four 240 mg tablets) twice daily, about 12 hours apart, swallowed whole with water.
