How Does Ribociclib Work?

Ribociclib is a selective CDK4/6 inhibitor. Cyclin-dependent kinases 4 and 6 control the checkpoint that lets cells pass from the G1 phase into the DNA-synthesis phase of the cell cycle. HR-positive breast-cancer cells rely on this pathway to keep dividing, so blocking CDK4 and CDK6 halts them at the checkpoint and slows tumour growth. When paired with hormone therapy that suppresses oestrogen signalling, the combination attacks the cancer from two complementary directions.

Indications

The approved indications are HR-positive, HER2-negative advanced or metastatic breast cancer in combination with an aromatase inhibitor or fulvestrant, and HR-positive, HER2-negative stage II/III early breast cancer at high risk of recurrence as adjuvant therapy. Treatment should be initiated and monitored by an oncologist, and hormone-receptor and HER2 status must be confirmed before starting.

Dosage

For advanced or metastatic disease the recommended dose is 600 mg (three 200 mg tablets) once daily, with or without food, in 28-day cycles of 21 days on and 7 days off. A 63-tablet pack therefore covers three weeks of the active-treatment phase. For early breast cancer the dose is 400 mg (two tablets) daily. The dose may be reduced to 400 mg or 200 mg daily to manage neutropenia, liver-enzyme elevations or QT prolongation.

Side Effects

The most common side effects are neutropenia, nausea, fatigue, diarrhoea, vomiting, hair loss, headache and constipation. Important but less frequent effects include QT-interval prolongation, liver toxicity, interstitial lung disease and severe skin reactions. Blood counts, liver function, electrolytes and the ECG are monitored regularly, and patients should report fever, persistent diarrhoea, yellowing of the skin or new breathing difficulty without delay.

Storage

Store Kisqali tablets at room temperature in the original packaging, protected from moisture, and out of the reach of children. Do not use after the expiry date printed on the pack.
