Stivarga is the originator brand of regorafenib, developed and marketed worldwide by Bayer of Germany. What Is Stivarga Used For?

It is used in metastatic colorectal cancer that has progressed despite fluoropyrimidine-, oxaliplatin- and irinotecan-based chemotherapy plus anti-VEGF and, where relevant, anti-EGFR treatment. It is also indicated for gastrointestinal stromal tumour that has become resistant to imatinib and sunitinib, and for hepatocellular carcinoma that has progressed on sorafenib. In all three settings Stivarga is a later-line option for disease control.

How Does Regorafenib (Stivarga) Work?

Regorafenib is an oral inhibitor of multiple kinases involved in tumour growth and angiogenesis. It blocks the VEGF receptors 1, 2 and 3, TIE2, PDGFR-α and β, FGFR-1 and 2, KIT, RET, RAF-1 and BRAF. This broad inhibition suppresses both the signalling that makes cancer cells divide and the formation of new blood vessels that feed the tumour, which together slows disease progression in several cancer types.

Indications

The approved indications are metastatic colorectal cancer after standard therapy, imatinib- and sunitinib-refractory gastrointestinal stromal tumour, and sorafenib-refractory hepatocellular carcinoma. Stivarga should only be prescribed by an oncologist experienced in these diseases, and patients require assessment of liver function and blood pressure before and during treatment.

Dosage

The standard dose is 160 mg, taken as four 40 mg tablets once daily with a low-fat meal. Treatment is given in 28-day cycles consisting of 21 days on therapy followed by 7 days off. A 28-tablet pack supplies seven days of active dosing. Dose reductions to 120 mg or 80 mg daily are common when side effects such as hand-foot skin reaction or liver enzyme rises occur, and the dose may be escalated again once symptoms improve.

Side Effects

Common side effects include hand-foot skin reaction, diarrhoea, fatigue, hypertension, decreased appetite, and mucositis. More serious adverse events can include severe hepatotoxicity, haemorrhage, gastrointestinal perforation and cardiac ischaemia. Regular monitoring of liver enzymes, blood pressure and full blood counts is essential, and patients should promptly report skin peeling on the hands or feet, persistent diarrhoea, dark stools, or chest pain.

Storage

Store Stivarga tablets at room temperature in the original container, away from moisture, and out of the reach of children. Do not use past the expiry date shown on the pack.
